Every patient at the office receives a comprehensive health history, orthopedic, neurological and musculoskeletal examination to specifically determine the cause of your condition. If additional tests are required, such as x-rays, MRI, nerve conduction studies, and/or laboratory tests—the doctor will refer you to a local facility to have those performed. Treatment plans will be developed according to your individual needs and a thorough report of my findings will be explained so that you can understand exactly what you have and how we are going to treat it.

Please bring the following to your first visit:

1. Recent and past relevant diagnostic testing:
2. Xrays, MRI, MRA, CT scan, NCV/EMG and EEG reports
3. Blood work, urine analysis, food and allergy test results
4. Organize the above in chronological order and place in a binder
5. Mail or drop off 1 week prior to your visit if you have a complex case
6. Filled out new patient forms

Note: We do not need doctor’s clinical notes.  Only diagnostic test reports on a CD  or hard copy images and examination reports.
